BRIEF DESCRIPTION OF POSITION:
The case manager will provide direct services to clients at the Family Resource Center and assist the Family Resource Specialist as necessary including referrals, intakes, case management and evaluation. Case Manager will provide services to families in the San Bernardino East Valley through Family Resource Centers based at Title 1 Elementary Schools.

QUALIFICATIONS

EDUCATION: Bachelor¡¦s degree in social work, psychology, liberal studies, marriage and family therapy, or closely related field

EXPERIENCE & 1-3 years experience in developing, implementing, and managing social
KNOWLEDGE: programs in a community setting; non-profit organizational experience a plus
„P Experience, knowledge, and/or education in case management
„P Must have or possess the ability to learn of community resources
„P Experience and/or education in child development
„P Demonstrate ability to organize and track data for program outcomes
„P Strong professional and interpersonal communication skills
„P Fluency in Spanish desired, not required

ABILITIES: Work effectively in a high-paced environment, involving the ability to:

„P Demonstrate familiarity with non-profit organizations and community based organizations
„P Maintain a friendly environment at the Family Resource Center
„P Successfully track a client¡¦s progress through case management
„P Establish rapport with client population, access needs of client, and effectively create reachable goal plans for client
„P Drive a car and the means to provide own transportation when required
„P Make recommendations to ensure the effective performance of Family Support Programs
„P Assist in the mission of Building A Generation¡¦s Family Support Program.
„P Help clients identify and utilize their strengths when accessing community resources

PERSONAL
QUALIFICATIONS: Character and proper social capability to
„P Relate effectively with racially and ethnically diverse staff and community
„P Work with a wide variety of community groups and organizations
„P Work independently, with self-initiative and personal integrity



Salary $33,280.00 ¡V $36,025.60 annually for a grant funded position. ¡V (Full Time, M-F 8:00-4:30pm with willingness to work occasional evenings and weekends)


DUTIES AND RESPONSIBILITIES:

o Assist Family Resource Center Specialist and Director as needed in general program operations.

o Manage a caseload of up to but not limited to 20 clients

o Conduct comprehensive client assessment to develop appropriate service plans

o Provide crisis intervention to clients when necessary

o Maintain timely documentation of services (including case files, client outcomes, and agency technical reports)

o Develop and nurture relationships with center¡¦s site personnel, families, referral agencies, and integral community members.

o Collect and record evaluation data under the direction of the Director, assist in implementing and using evaluation tools.

o Demonstrate understanding of the nature of and operations of a non-profit organization.

o Participate in organization-wide growth and development activities
